I'm also having trouble figuring out how the datepicker. I've tried two methods. First, I can replicate the datepicker layout just as you have it, but am not sure how to tie it to an input field.
Second, I can also set up a simple datapicker using default jqueryUI but cannot get the popup datepicker formatted in the format you achieve.
